Application Forms for Apply

There is no cost to apply to Apostles. Fill out the application and teacher recommendation forms below. Please submit the forms to the school office. The application should be submitted online; the recommendation form can be mailed or faxed.

Age requirements for enrollment:

• To enter our K1 program, children are generally five years old before the end of the school year.
• To enter our K2 program, children must be 5 years old on or before September 2nd.
• To enter first grade, children must be 6 years old on or before September 2nd.
